The Antithesis of EMR

Adventures in Medicine posted yet another great blog last week on where charting has gone. As you sort throught the EMR landscape and review the details of blood pressures from 3 years ago on you're patients consider this. They found a chart dating back 50 years with a simple entry:

"Hospitalized for bacterial meningitis. Fully recovered"


Do you really need to know anything more?
